Homemaker & Extraordinary Cleaning

Individual Residential Services and Supports

Individuals in this waiver program requires 24-hour residential support and habilitation in community settings. It covers personalized care in the individual home, the caregiver home, agency-owned homes, or host homes, focusing on safety, skill-building, and community inclusion.

These services are designed to support individuals with disabilities by providing specialized cleaning, disinfection, and sanitization services. These services are necessary to ensure a safe, hygienic living environment and prevent the spread of infectious diseases or pathogens. The Extraordinary Cleaning component includes the use of commercial-grade products, infection control protocols, handling and disposal of biohazard materials, and specialized materials or equipment. These services are beyond routine homemaking tasks and are required due to or as a result of a Member's disability.

Mentorship

Goal: To promote independence, self-advocacy, and community participation.

Activities: Mentors instruct and advise on tasks like interviewing providers, navigating transportation, and managing personal affairs.

Service Delivery: This is often available through the Family Care Giver service model.

Context: Mentorship is designed to complement, not replace, natural supports and is tailored to the individual's specific, unmet needs within their own home or community.

Related Services: It may be paired with peer mentorship for those transitioning to the community or with specific caregiver education.

This service provides door-to-door transit for Health First Colorado (Medicaid) members to essential, non-health-related locations, such as adult day programs, shopping, and community integration activities.

These services focus on Activities of Daily Living (ADLs) and are typically delivered in the member's home:

Hygiene and Grooming: Assistance with bathing, hair care, shaving, nail care, and dressing.

Toileting: Help with bladder and bowel care, including incontinence supplies.

Mobility and Transfers: Support with walking, moving from a bed to a chair, or using specialized equipment like Hoyer lifts.

Nutrition: Meal planning, preparation, and assistance with eating.

Health Maintenance: Medication reminders (non-administration) and help with exercises or respiratory equipment.

Household Support: Light housekeeping, laundry, and grocery shopping related to the member's care.
